Through our Woven Connect™ software, Weavers™ and Care Managers are able to log and monitor daily activities with our clients.
Rapid alerts
If a concerning entry is added to a client's Woven Connect™ profile (e.g. an increased fall risk or a new symptom), our Care Managers will be alerted.
Family collaboration
A client's selected loved one will also have access to Woven Connect™ to continue monitoring at-home changes, allowing them to be an informed partner in making future decisions.
Flexible scheduling
Everyone has a different schedule and different levels of needs. Our Weavers™ are flexible and Woven Home Care™ will coordinate with the client to find the best cadence for our relationship.
